Your First Appointment

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options.

IMPORTANT: Please alert the office if you have a medical condition or are taking medications.

A parent or guardian must accompany all patients under 18 at the consultation visit.

Please arrive 20 minutes early to complete patient forms. Payment is due at the time of service.
