Insurance
As a courtesy, we will file claims on your behalf. However, patients are responsible for any remaining balances not covered by insurance. Please provide accurate and up-to-date insurance information during your visit.
Payment Responsibilities
All fees are due at the time of service unless prior arrangements have been made. For extensive treatments, a financial agreement may be established to outline payment terms.
Missed Appointments
